Integrated Health Care Manager

General Description

The Integrated Health Care Manager coordinates plans of service for individuals who have been identified as having chronic health problems as well as mental illness and/or substance use disorders, or developmental disorders. He/she participates in multiple interdisciplinary teams and provides them with information and referrals (education about mental illness, substance use disorders, and developmental disorders), and is a liaison between the Family Medical Center (a Federally Qualified Health Clinic) and community providers.

The Integrated Health Care Manager also provides consultation and crisis stabilization services for urgent and emergent mental health and/or substance use situations that present in the populations served at the Family Medical Clinic. He/she assists in the development of wellness education.

Minimum Qualifications

  • Master's degree in Social Work, Psychology, or other related clinical degree; certification or licensure in the State of Michigan is required.
  • At least two years' experience providing services to individuals with severe psychiatric or substance use disorders is required.
  • Substance use treatment certification is preferred.

Competencies

  • Understanding of mental health, substance use disorders and developmental disorders and their impact on health is required.
  • Ability to interact with physicians and other health care professionals in a professional manner.
  • Working knowledge of physical health and behavioral health medications.
  • Ability to use health data (lab, hospital, and Emergency Room reports) in developing an interdisciplinary treatment plan.
  • Knowledge and ability to use screening assessments for mental illness, suicidality, developmental disorders, and substance use disorders.
  • Demonstrated competence in assessment and referral for behavioral health, developmental disorders, and substance use disorders.
  • Ability to provide crisis assessments and referrals in the primary care clinic.
  • Knowledge of community resources.
  • Thorough knowledge and ability to use the Electronic Health Record for documentation of plan of service and progress towards goals.
  • Training and demonstrated competence in Motivational Interviewing, Trauma Informed Practices, and Brief Solution Focused Interventions.
  • Demonstrated ability to provide group support and education about behavioral health, developmental disorders, and substance use disorders.
  • Familiarity with managed care, state, federal, and private health insurance benefits.
  • Excellent interpersonal communication skills and the ability to interact effectively with staff and patients are desirable.
  • Excellent written communication and grammatical skills are required.
  • Attention to detail and excellent organizational skills are required.
  • The ability to demonstrate absolute discretion with confidential information is required.
  • Experience with Microsoft Office applications (Word, Excel, Power Point, etc.) is highly desirable.
  • Experience using office equipment (including, but not limited to, desktop telephone, computer, copier, fax machine, scanner, laser printer, and desktop calculator).

Essential Duties and Responsibilities

  1. Provide Care Management Services for patients identified as having severe chronic health conditions and severe mental illness, substance use disorders or developmental disabilities.
  2. Provide emergency mental health assessment and referrals at the Family Medical Center.
  3. Provide "curbside counseling" / crisis management to patients at the Family Medical Center.
  4. Provide brief intervention to stabilize crisis and plan for ongoing support.
  5. Act as liaison between the Family Medical Center and Lenawee Community Mental Health Authority.
  6. Provide accurate and efficient support as needed to LCMHA staff (i.e.: connecting to community supports, communicating with Family Medical Center staff).
  7. Provide mental health consultation to primary health care providers.
  8. Assist primary health care providers with identification and referral of patients screened as needing additional behavioral health services.
  9. Collect and monitor health care data for reporting and analysis.
  10. Participate on Integrated Health Care team.
  11. Co-facilitate health and wellness activities.
  12. Demonstrate a person-centered approach to assessment and treatment.
  13. Complete documentation as required to meet standards for timeliness and accuracy.
  14. Carry out additional duties as assigned by supervisor.

Other Duties and Responsibilities

  • Maintain (at employee expense) state license to provide professional services in Michigan.
  • Attend LCMHA mandatory in-service training.
  • Comply with all LCMHA policy and procedures.
  • Maintain positive working relationships with agencies in the community.
  • Work cooperatively with program staff and across programs at LCMHA and the Family Medical Clinic.

Physical Demands and Work Environment

Duties require sufficient mobility to work in a normal office setting and use standard office equipment including a computer, vision to read printed materials and a computer screen and hearing and speech sufficient to communicate in person or over the telephone.

Should be able to bend, stretch, reach, and lift up to 20 pounds. An individual in this position needs to move about inside the office occasionally to access file cabinets, office machinery, etc. The work environment involves sitting most of the time in an office environment infrequently exposed to extreme temperature, noise, fumes, dust, etc.

Special Position Requirements

This position requires possession of a valid U,S. driver's license or method of transportation to travel within Lenawee County and to meetings outside of the county when requested.
